BGS Reslab Question

I recently received my order and one of the cases has a loose piece of plastic. I emailed BGS and they said they would provide a new case at no charge. I also have a previously graded card that I would like to get reslabbed, which they said is $5. The email said I need to complete a submission form like I would for any other card and send everything together with a copy of the email. How exactly would I fill out the form? Not sure how to go about this. Thanks.
