No Rip Card In Topps

Well I didn't get a rip card in my case. What do I need to send in so I can get a rip card. I still have all the boxes and 5 boxes worth of wrappers. Any help is appreciated.

upc codes from each box, a receipt...... might as well send the wrappers for good measure. let Topps deal with them.

also, include a note describing what was missing.
