Replacing BGS Slab

BO Hivemind,

I need to replace a BGS slab that is damaged (damage does not extend into the card "pocket") and can't get BGS to return my emails. Does anyone know where to go on their site to initiate a "reslab"? Is there a form that I just can't find?

In my experience, just fill out a sub form and mark “Re-case” on the right margin. Also put a sticky note on the slab for confirmation. It is $8. Used to be $5 but changed in October for a crack out fee.
