One of my first thoughts is how does this play in with Ohtani? Let's say Trout's back is mostly manageable. Angels would still want to proceed with caution. Move him to a corner outfield spot, give him more scheduled days off, no double headers, etc.
I would think the rest would also involve a heavy dose of him DHing to preserve his bat in the lineup. Ohtani currently does DHing. Managing both of these and keeping them both in the batting lineup is going to be tricky. If the Angels resign Ohtani, both these players have huge payrolls and are your top two hitters. Obviously, your chance of actually making the playoffs mean both are putting up their usual good numbers. Something like Trout DHs on Ohtani rest days.
